+- Source changes: restrict GOAWAY(NO_ERROR) classification to Hyper's peer-
+  initiated debug variant. Recycle exactly the observed upstream pool generation
+  and replay only one empty GET/HEAD/OPTIONS request. No unsafe/body replay.
